Patient Information

Office Hours

In general, our office hours are 8:30 to 5:15 from Monday to Friday.  Our office telephones are staffed Monday to Friday, 9:00 AM until 1:00 PM and from 2:00 PM until 5:00 PM.

Please call 911 if you have a life threatening emergency. If you go to the Emergency Room, Dr. Yamaki practices at St. Joseph Hospital in Tacoma. If you have an urgent or non-emergent issue after hours, please call the Answering Service at (253) 922-4805, They can have you speak with the doctor on call, but please be aware there may be a charge for non-urgent services requested through the Answering Service.

After Hours

Non-urgent phone messages may be left on the office phone when the office is closed.  When leaving a message please slowly and clearly state your full name, your date of birth and repeat your phone number twice.

Appointment Reservations, Cancellations and No-Show Policy

In fairness to us and patients who are anxious to be seen sooner, there is a charge for missed appointments and cancellations made without providing us more than one full work day notice.  New patients wishing to make an appointment will be asked for a credit card to hold their appointment time.  The patient’s card will reflect a hold but a charge is only applied if the patient no shows or cancels without providing adequate notice.

Prescription Refills

Please ask your pharmacy to send refill requests to us electronically or by fax to (253) 927-6911 and allow 2 full days for the request to be authorized.  We are able to refill prescriptions only if your medical history and care are current.

Test Results

Depending on the test, results should be available within 7 to 14 days. You will be notified about results either by phone, mail, or our Personal Patient Information Line. If you are asked to call the Personal Messaging Line, (800-536-3132), your password is your full social security number. Please call us if you have not heard about any test results and more than two weeks have lapsed. Please note your lab tests and other services performed outside of our office will be billed to you or your insurance by the respective facility.

We participate in many but not all insurance plans. Health Care Reforms are pushing many patients into restrictive plans which we are not affiliated with.  So Please ask about your insurance coverage when scheduling your appointment. Payment for co-payments, deductibles and balances are expected at the time of service. We accept cash, personal checks, Visa, MasterCard and CareCredit.
